Abstract

The clinical case of an 11-year-old female patient is reported, there is still no menarche and she is accompanied by her mother, stating that the patient looks "stubby" (increased over jet). The patient is instructed to carry out cabinet studies such as orthopantomography and lateral of the skull, oral and extraoral photographs are taken, exploration and a later appointment is given. At 4 weeks later and with the help of radiographs and oral photographs we found: Mesofacial patient, no asymmetry is observed in three thirds, the lines corresponding to the rule of fifths we observe symmetry of all, a positive smile is observed, showing the anterior teeth up to the middle third, a convex profile and lip competence is observed, it does not present absence of dental organs, on its right lateral side it presents a molar class I and canine class I and on its left lateral part it is class II molas and canine class II, presents an oval arch in the upper jaw, as for the mandible there is a wide ovoid dental arch, caries in dental organs 16, 36 and 46, crowding in the lower anterior area, oral tissues and healthy periodontal bones, having the data we can diagnose the patient with an increased over jet, a mandibular retrognathism, skeletal Class II, presenting ma alveolar dental protrusion xillary, a maxillary alveolar dental proclination, a mandibular dental protrusion and a mandibular dental proclination, tendency to vertical growth, tendency to open bite and biprochelia, its possible treatment plan is studied and discussed since it is in the age limit and is requires a combination of maxillary orthopedics and orthodontics.

Se reporta el caso clínico de una paciente femenina de 11 años de edad, aun no se presenta menarca y es acompañada por su mamá refiriendo que la paciente se ve “trompuda” (over jet aumentado) se le indica a la paciente realizar estudios de gabinete tales como ortopantomografía y lateral de cráneo, se realizan las fotografías orales y extraorales, exploración y se le da cita posterior. A las 4 semanas posteriores y con la ayuda de las radiografías y fotografías orales encontramos: Paciente mesofacial, no se observa una asimetría en los tres tercios, los trazos correspondientes a la regla de los quintos observamos simetría de todos, se observa una sonrisa positiva, mostrando los dientes anteriores hasta el tercio medio, se observa un perfil convexo y competencia labial, no presenta ausencia de órganos dentales, en su lado lateral derecho presenta una clase molar I y clase canina I y en su parte lateral izquierda es clase molas II y clase canina II, presenta un arco en forma oval en el maxilar superior, en cuanto a la mandíbula se observa un arco dentario amplio de forma ovoide, caries en los órganos dentales 16, 36 y 46, apiñamiento en zona anterior inferior, tejidos bucales y periodontales sanos, teniendo los datos podemos diagnosticar a la paciente con un over jet aumentado, un retrognatismo mandibular, Clase II esqueletal, presentando protrusión dental alveolar maxilar, una proclinación dental alveolar maxilar, una protrusión dental mandibular y una proclinación dental mandibular, tendencia a crecimiento vertical, tendencia a mordida abierta y biproquelia, se estudia y discute su posible plan de tratamiento ya que se encuentra en el limite de edad y se requiere combinación de ortopedia maxilar y ortodoncia.
