Manejo de discrepancia transversal en adulto joven, aplicación de protocolo Marpe

Abstract

Rapid palatal expansion is the procedure of choice to restore the transverse skeletal dimension in adult patients, through the association of orthopedic and dental effects, which consists of the biomechanical principle of separating the two maxillary halves by remodeling the palatal suture and intermaxillary sutures. Miniscrew-assisted rapid palatal expansion (MARPE) proposed by Lee, with the aim of resolving undesirable dentoalveolar effects and optimizing the potential for skeletal expansion in individuals in advanced stages of skeletal maturation. A 20-year-old female patient went to the Orthodontics clinic of the University Center for Postgraduate Studies and Research of the Michoacana University of San Nicolas de Hidalgo, her reason for consultation was "I have a bad bite." No significant findings are reported during the clinical history. There were no TMJ symptoms. Materials and methods: The treatment began by placing a 10 mm MSE appliance with 4 miniimplants 1.8x11 mm, five microperforations in the midline and the bands cemented to the first molars. Results: The clinical inspection of the patient is carried out at the end of the treatment where the success of the transverse expansion of the midpalatal suture can be observed. Another clinical sign that was observed was the interincisor gap of the upper central teeth. Observing the CT of the post-expansion maxilla, we observed a clear orthopedic separation of the median raphe from the midpalatal suture. Conclusions: Rapid palatal expansion assisted with microscrews in combination with microperforations, turns out to be effective in the treatment of adult patients with maxillary transverse collapse.

La expansión palatina rápida es el procedimiento de elección para restablecer la dimensión transversal esquelética en pacientes adultos, mediante la asociación de efectos ortopédicos y dentales, que consiste en el principio biomecánico de separar las dos mitades maxilares mediante la remodelación de la sutura palatina y las suturas intermaxilares. La expansión palatal rápida asistida por minitornillo (MARPE) propuesta por Lee, con el objetivo de resolver los efectos dentoalveolares indeseables y optimizar el potencial de expansión esquelética en individuos en etapas avanzadas de la maduración esquelética. Paciente femenino de 20 años de edad, acude a la clínica de Ortodoncia del Centro Universitario de Estudios de Posgrado e Investigación de la Universidad Michoacana de San Nicolas de Hidalgo, su motivo de consulta “Tengo mal la mordida”. No se informan hallazgos significativos durante la historia clínica. No había síntomas de ATM. Materiales y métodos: Se inicio el tratamiento colocando un aparato MSE de 10 mm con 4 Miniimplantes de 1.8x11 mm, cinco microperforaciones en línea media y las bandas cementadas a primeros molares. Resultados: Se realiza la inspección clínica del paciente al finalizar el tratamiento en donde se puede observar el éxito de la expansión transversal de la sutura media palatina. Otro signo clínico que se observó fue el diastema interincisivo de los centrales superiores. Observando la TAC del maxilar superior post expansión, observamos una clara separación ortopédica del rafe medio de la sutura media palatina.
