Efecto del apoyo educativo de enfermería para disminuir la incertidumbre en padres de RNP en UCIN

Abstract

Introduction. The Parents say that the hospitalization of their premature newborn was unexpected and surprising, causing them uncertainty and requiring educational support; 15 million premature newborns are born annually, most of whom require hospitalization in Neonatal Intensive Care Units (NICU). Objective. To assess the effect of educational support in decreasing the uncertainty of parents of preterm infants (RNP) in nicu. Material and methods. Quantitative study, quasi-experimental, longitudinal design and pre- and post-postest measurements in the experimental group; non-probabilistic sample of 30 parents of RNP. Mishel's disease uncertainty scale, 31 items, four factors, Likert scale of 1-5 and three categories. Educational intervention of 4 weekly sessions of 25 minutes. Descriptive and inferential statistics (Student's t). Results. In RNP, most men (70%), mean gestational age of 29.6 (ED=3.65) weeks and 1.32 kg (ED=0.59) weight, with respiratory distress syndrome (93%) and mechanical ventilation (60%). In mostly married parents (66.7%), women (66.7%), secondary schooling (36.7%) and mean age 25.8 (DE=7.46) years. A decrease was observed in categories of uncertainty in total scale and dimensions in the postest, with statistically significant differences in the total scale and in dimensions (p<.05) except in Unpredictability. Conclusions. The educational support implemented in RNP parents was effective in depressing the uncertainty before the disease except in the inability of the parents to make predictions about their child's health prognosis.

Introducción. Los padres/madres expresan que fue algo inesperado y sorprendente la hospitalización del recién nacido prematuro causándoles incertidumbre por lo que requieren apoyo educativo; anualmente nacen 15 millones, la mayoría requieren hospitalización en las Unidades de Cuidados Intensivos Neonatales (UCIN). Objetivo. Evaluar el efecto del apoyo educativo para disminuir la incertidumbre de padres de recién nacidos prematuros (RNP) en UCIN. Material y métodos. Estudio cuantitativo, diseño cuasiexperimental, longitudinal y mediciones pre y postest en grupo experimental; muestra no probabilística de 30 padres/madres de RNP. Escala de incertidumbre ante la enfermedad de Mishel, 31 ítems, cuatro factores, escala Likert de 1-5 y tres categorías. Intervención educativa de cuatro sesiones semanales de 25 minutos. Estadística descriptiva e inferencial (t de Student). Resultados. En RNP mayoría hombres (70%), media de edad gestacional de 29.6 (DE=3.65) semanas y 1.32 kg (DE=0.59) de peso, con síndrome de dificultad respiratoria (93%) y ventilación mecánica (60%). En padres mayoría casados (66.7%), mujeres (66.7%), escolaridad secundaria (36.7%) y media de edad 25.8 (DE=7.46) años. Se observó disminución en categorías de incertidumbre en escala total y dimensiones en el postest, con diferencias estadísticamente significativas en la escala total y en dimensiones (p<.05) excepto en Imprevisibilidad. Conclusiones. El apoyo educativo implementado en padres de RNP fue efectivo para disminuir la incertidumbre ante la enfermedad excepto en la imposibilidad de los padres para hacer predicciones ante el pronóstico de salud de su hijo.
