Control de hábitos en deglución atípica en el tratamiento de mordida abierta, caso clínico

Abstract

BACKGROUND: A habit is defined as the repetition of an action, which must always be the same, constant and easy to perform. There are habits classified as harmful that can have negative consequences for a patient who is developing. The habit of atypical swallowing can cause anterior open bite and has different ways of being treated. The Simoe Network 3 appliance has proven to be an alternative to efficiently control this habit and thus avoid skeletal malformations that affect aesthetics and function. In order to have a management against the habit of atypical swallowing, the age, level of skeletal development, dental eruption and facial tissues of the patient must be carefully evaluated, as well as a comparison with other functional appliances to obtain the best possible result. OBJECTIVE: To describe the options that exist for the treatment of anterior open bite caused by the harmful habit of atypical swallowing. MATERIALS AND METHODS: A patient was selected from the maxillary orthopedic clinic at the postgraduate studies center of the Michoacana University of San Nicolás de Hidalgo who had the characteristics of developing an anterior dental and skeletal open bite due to an atypical swallowing habit. Panoramic and lateral skull radiographs, cephalometric studies, measurements on plaster study models, and analysis of intraoral and extraoral photographs were performed to obtain a diagnosis. A functional orthopedic treatment plan was established to treat the harmful habit and thus prevent the development of the anterior open bite. RESULTS: By using the Simoe Network 3 (SN3) appliance, a better overbite was observed in the progress, along with a correct eruption of the upper central incisors, since the harmful habit of atypical swallowing was removed and therefore the anterior open bite disappeared.

ANTECEDENTES: un hábito se define cuando existe la repetición de una acción, debe ser igual siempre, constante y fácil de realizar. Existen hábitos catalogados como nocivos que puede traer consecuencias negativas en un paciente que se encuentra en desarrollo. El hábito de deglución atípica puede provocar mordida abierta anterior y tiene distintas maneras de tratarse. El aparato Simoe Network 3 ha demostrado ser una alternativa para controlar este hábito de manera eficiente y así evitar malformaciones esqueletales que afectan la estética y función. Para poder tener un manejo contra el hábito de deglución atípica se deben evaluar minuciosamente la edad, nivel del desarrollo esqueletal, erupción dental y tejidos faciales del paciente, así mismo realizar una comparación con otros aparatos funcionales para obtener el mejor resultado posible. OBEJTIVO: describir las opciones que existen para el tratamiento de la mordida abierta anterior que se ocasiona por el hábito nocivo de deglución atípica. MATERIALES Y METODOS: se seleccionó a un paciente de la clínica de ortopedia maxilar en el centro universitario de estudios de posgrado de la Universidad Michoacana De San Nicolás De Hidalgo que tenía las características de estar desarrollando una mordida abierta anterior dental y esqueletal debido al hábito de deglución atípica. Se realizaron radiografías panorámica y lateral de cráneo, estudios cefalométricos, medición en modelos de estudio en yeso y análisis de fotografías intraorales y extraorales para obtener un diagnóstico. Se estableció un plan de tratamiento ortopédico funcional para tratar el hábito nocivo y así evitar desarrollo de la mordida abierta anterior. RESULTADOS: al usar el aparato Simoe Network 3 (SN3) se observó en el avance un mejor overbite, junto con una erupción de incisivos centrales superiores de manera correcta, ya que se quitó el hábito nocivo de deglución atípica y por lo tanto desapareció la mordida abierta anterior.
