Funcionalidad familiar en gestantes adolescentes atendidas en consulta externa de medicina familiar

Abstract

INTRODUCTION: Teenage pregnancy is a public health problem due to its high morbidity and mortality. Observing a relationship between pregnancy at a young age and family dysfunction secondary to a poor relationship between fathers and daughters, seeking the protection of the absent father figure in the romantic couple. GENERAL OBJECTIVE: To evaluate family functionality in pregnant adolescents and its relationship with pregnancy in the context of family medicine outpatient clinics. MATERIAL AND METHODS: Descriptive cross-sectional study of 66 pregnant adolescents assigned to HGZ/MF No.2, from January to June 2024, who agreed to participate in the study. Patients with incomplete questionnaires were excluded. The “FF” test was applied for family functioning. Descriptive statistics and chi2 were used for association of qualitative variables with a significant value p<0.05. RESULTS: 66 pregnant women were surveyed, with a mean age of 18.4±1 year, marital status predominated: married 31 (47.0%), secondary education 41 (62.1%) and primigravida 49 (74.2%). Functional family 2(3.0%) presented, mild dysfunction 19(28.8%), moderate dysfunction 43(65.2%), severe 2(3.0%). The role domain had the greatest impact 58 (87.9%), followed by communication 56 (84.8%) and affective response 41 (62.1%). An association was found between family functionality and education [p <0.001]. CONCLUSIONS: The majority of pregnant adolescents present some degree of family dysfunction, with moderate dysfunction being the most prevalent; with greater involvement of the domains of roles, communication and affective response in their family environment, therefore, it is important to address the underlying family dynamics in addition to their medical needs.

INTRODUCCIÓN: El embarazo adolescente es un problema de salud pública por la elevada morbimortalidad. Observándose existe relación entre el embarazo a corta edad y la disfunción familiar secundario a una mala relación entre padres-hija, buscando la protección de la figura paterna ausente en la pareja sentimental. OBJETIVO GENERAL: Evaluar la funcionalidad familiar en gestantes adolescentes y su relación con el embarazo en el contexto de la consulta externa de medicina familiar. MATERIAL Y MÉTODOS: Estudio transversal descriptivo, a 66 gestantes adolescentes adscritas al HGZ/MF No.2, de enero a junio 2024, que aceptaron participar en el estudio. Se excluyeron pacientes con cuestionarios incompletos. Se aplicó el test “FF”, para funcionamiento familiar. Se utilizó estadística descriptiva y chi2 para asociación de variables cualitativas con valor significativo p<0.05. RESULTADOS: Se encuestaron 66 gestantes, con edad media 18.4±1 año, predominó estado civil casada 31(47.0%), escolaridad secundaria 41(62.1%) y primigestas 49(74.2%). Presentó familia funcional 2(3.0%), disfunción leve 19(28.8%), disfunción moderada 43(65.2%), severa 2(3.0%). El dominio de roles tuvo mayor afectación 58(87.9%), seguido de comunicación 56(84.8%) y respuesta afectiva 41(62.1%). Se encontró asociación entre funcionalidad familiar con escolaridad [p <0.001]. CONCLUSIONES: La mayoría de las gestantes adolescentes presentan algún grado de disfunción familiar, siendo la moderada la de mayor prevalencia; con mayor afectación de los dominios de roles, comunicación y respuesta afectiva en su entorno familiar, por tanto, es importante abordar las dinámicas familiares subyacentes además de sus necesidades médicas.
