Abstract: The appropriate technique to insert a catheter into peripheral or central venous vessels is basic in the management of the patients, the treatment involve to remain in the hospital for period of time more tan one week, because of this appear the necessity of central venous catheterization. The human body have numerous access to central venous catheterization, the preferred technique for installation is that describer by Seldinger and the vein dissection, all of this are not exempt of complications. Nowadays puncture can be performed under ultrasonographic view being gold standard because of the significant reduction in complications. In our investigation, we will use the ultrasonographic technique to resolve the problem of central venous catheterization by percutaneous puncture. Material and methods. Type of study: experimental, longitudinal, analytical, prospective and with minimun risk. Results. We collect a total sample of 85 patients to apply catheter by the ultrasonographic method, 50 (59%) patients were man 35 patients were woman (41%) of wich 57 (67%) were located in the right internal jugular vein, in the left internal yugular vein were located 17 (20%), finaly femoral vein, subclavian vein and axillary vein were 11 (13%), we established the usefulness of the US Doppler, determinating how deep in the vein and the depth of the puncture. About the complications that our patients presented were hematoma and local edema in 14 case (16.4%), vein thrombosis in 2 case (2.4%), arterial puncture 0 case (0%) and in 69 case we did not have complications (81%). The numbers of attempts was an important part of our investigation, in one puncture we put 69 catheters (81%), 14 cases in two punctures (16.5%) and in 3 or more puncture were 4 case.
La adecuada canalización de vasos venoso periféricos y centrales es básica en el diario manejo del paciente, la terapéutica implica estancias intrahospitalarias prolongadas, uso de medicamentos flebotóxicos resulta insuficiente la canalización venosa periférica, surge la necesidad de colocar catéter venoso central. El cuerpo humano cuenta con accesos venosos centrales los cuales susceptibles de canalizar. Las complicaciones inmediatas a la colocación de catéter venoso central son múltiples y pueden llegar a poner en peligro la vida del paciente. Existen varias técnicas para la colocación del catéter venoso central, todas basadas en Técnica Seldinger, venodisección entre otras, ninguna exenta de complicaciones, se encuentra ya descrito el uso de colocación de catéter venoso central guiado por ultrasonido, en nuestra investigación se utilizara esta técnica para resolver el problema. Material y métodos. Tipo de estudio: Experimental, longitudinal, analítico, prospectivo y con riesgo mínimo. Resultado. Una muestra total de 85 pacientes, en quienes colocamos 85 catéteres por medio de ultrasonido Doppler, 50 a masculinos (59%) y 35 a femeninos (41%), de los cuales 57 (67%) fueron en yugular interna derecha, yugular interna izquierda 17 (20%), femorales, subclavios y axilares 11 (13%), establecimos la utilidad del US Doppler a determinar la profundidad de punción, valoración del diámetro del vaso a canalizar y guiar en tiempo real el momento de la punción, de las complicaciones que presentaron nuestros pacientes fueron hematoma y edema local en 14 casos (16.4%), trombosis venosa 2 casos (2.4%), punción arterial 0 y en 69 de los casos no se registró complicación, 69 (81%) catéteres se colocaron en 1 punción, 14 (16.5%) en 2 punciones y 4 (2.5 %) en 4 punciones.
