Abstract: To analyze the epidemiological clinical profile of the invasive pneumococcal disease treated at the Children's Hospital of Morelia "Eva Sámano de López Mateos" during the period January 2008 to July 2017. Type of study: A retrospective, observational, cross-sectional and descriptive study. Material and methods: All patients admitted to the Children's Hospital of Morelia, from January 2008 to July 2017, with a confirmed diagnosis of invasive pneumococcal disease (meningitis, sepsis / bacteremia, complicated pneumonia with pleural effusion or bacteremia, invasive infection) were included. in another place outside the respiratory tract) by isolation of Streptococcus pneumoniae in cerebrospinal fluid, pleural fluid, blood culture, bronchial aspirate and other normally sterile site outside the respiratory tract. Two study periods were considered based on the beginning of the application of PCV13 in March 2012, considering 2008-2011 prior to its application, and 2012-2017 after its universalization. Results: 40 patients were studied, 20 males and 20 females, of which 33 had serotyping. An annual rate of 3 to 4 cases was recorded. The age group most affected was that of children under 2 years of age by 40%. The anatomic site with the highest isolation was blood, blood culture, in 27.5%. The distribution of the nosological entities was pneumonia 35%, sepsis 27.5%, neuroinfection 15%, and Other invasive infections (septic arthritis, abdominal sepsis, periorbital abscess, purulent conjunctivitis of the newborn, bacterial conjunctivitis, submaxillary abscess, pericarditis) in a 22.5 %. The most frequent serotypes were 19A (24.3%), 23F (12.2%), 6A (9.1%) and 6B (9.1%). The presence of serotypes not included in PCV13, serotype 33, 10B and 10, the latter two causing neuroinfection, as well as serotype 10A and 15B causing septic arthritis and abdominal sepsis, respectively, was demonstrated.
Analizar el perfil clínico epidemiológico de la enfermedad neumocócica invasiva atendida en el Hospital Infantil de Morelia "Eva Sámano de López Mateos" durante el periodo enero 2008 a julio 2017. Tipo de estudio: Un estudio retrospectivo, observacional, transversal y descriptivo. Material y métodos: Se incluyeron todos los pacientes internados en el Hospital Infantil de Morelia, de Enero de 2008 a Julio de 2017, con diagnóstico confirmado de Enfermedad neumocócica invasiva (meningitis, sepsis/bacteriemia, neumonía complicada con derrame pleural o bacteriemia, infección invasiva en otro sitio fuera del tracto respiratorio) por aislamiento de Streptococcus pneumoniae en líquido cefalorraquídeo, líquido pleural, hemocultivo, aspirado bronquial y otro sitio normalmente estéril fuera del tracto respiratorio. Se consideraron dos periodos de estudio basados en el inicio de la aplicación de PCV13 en marzo de 2012, considerando 2008-2011 previo a su aplicación, y 2012-2017 posterior a su universalización. Resultados: Se estudiaron 40 pacientes, 20 masculinos y 20 femeninos, de los cuales 33 contaban con serotipificación. Se registró una tasa anual de 3 a 4 casos. El grupo etario más afectado fue el de menores de 2 años en un 40%. El sitio anatómico de mayor aislamiento fue la sangre, hemocultivo, en un 27.5%. La distribución de las entidades nosológicas fue neumonía 35%, sepsis 27.5%, neuroinfección 15%, y Otras infecciones invasivas (artritis séptica, sepsis abdominal, absceso periorbitario, conjuntivitis purulenta del recién nacido, conjuntivitis bacteriana, absceso submaxilar, pericarditis) en un 22.5%. Los serotipos más frecuentes fueron 19A (24.3%), 23F (12.2%), 6A (9.1%) y 6B (9.1%). Se demostró la presencia de serotipos no incluidos en la PCV13, serotipo 33, 10B y 10, estos dos últimos causantes de neuroinfección, así como el serotipo 10A y 15B causantes de artritis séptica y sepsis abdominal, respectivamente.
