Detección de hipotiroidismo congénito primario en prematuros del Hospital Infantil de Morelia

Abstract

Congenital hypothyroidism (CH) is a thyroid hormone deficiency present at birth. HC is a preventable cause of mental retardation. The neurological prognosis depends on the timely and correct initiation of treatment. Objectives: To identify the incidence of congenital hypothyroidism in the preterm newborns treated at the Morelia Children´s Hospital during the period April 2018-November 2020. To estimate the incidence of congenital hypothyroidism in premature infants by gestational age and birth weight. Estimate incidence de congenital hypothyroidism by sex, as well as Assess the results of the screening and the thyroid profile performed in preterm newborns treated in the NICU service. Material and methods: This is a descriptive, retrospective and risk – free study. All premature newborns hospitalized in the neonatal intensive care unit of the Children´s Hospital of Morelia in the period from April 1, 2018- November 30, 2020 were included. The data obtains were analyzed using descriptive statistics. Results: 2 patients with a confirmed diagnosis of Congenital Hypothyroidism (incidence 1.98%), 1 woman (50%) and 1 man (50%), female/male ratio 1:1, were registered. 100% of the cases were late preterm, and 100% of the cases with low weight for gestational age. The neonatal screening was performed in 63% of the patients and the thyroid profile in 18% of the patients. Conclusions: The incidence of Congenital hypothyroidism in premature newborns at the Morelia Children´s Hospital is 1.98 per 100 hospitalized in the neonatal intensive care service. It was observed that the confirmed cases of congenital hypothyroidism were in the group of late preterm newborns of 36 weeks of gestation, as well as those with low weight for gestational age.

El hipotiroidismo congénito (HC) es la deficiencia de hormonas tiroideas presente al nacimiento. El HC es una causa de retraso mental prevenible. El pronóstico neurológico depende del inicio oportuno y correcto del tratamiento. Objetivos: Identificar la incidencia del hipotiroidismo congénito en los recién nacidos prematuros atendidos en el Hospital Infantil de Morelia durante el periodo abril 2018-noviembre 2020. Estimar la incidencia de hipotiroidismo congénito en prematuros por edad gestacional y peso al nacimiento. Estimar la incidencia de hipotiroidismo congénito por sexo, así como Valorar los resultados del tamiz y el perfil tiroideo realizado en los Recién Nacidos prematuros atendidos en el servicio de UCIN. Material y métodos: Se trata de un estudio descriptivo, retrospectivo y sin riesgo. Se incluyeron todos los recién nacidos prematuros hospitalizados en la unidad de cuidados intensivos neonatales del H. Infantil de Morelia en el período comprendido del 01 se abril 2018 al 30 de noviembre del 2020. Los datos obtenidos se analizaron de estadística descriptiva. Resultados: Se registraron 2 pacientes con diagnóstico confirmado de Hipotiroidismo Congénito (incidencia 1.98%) 1 mujer (50%) y 1 hombre (50%), relación mujer/hombre 1:1. 100% de los casos fue pre-término tardío, y el 100% de los casos con peso bajo para la edad gestacional. El tamiz neonatal se realizó en un 63% de los pacientes y el perfil tiroideo a un 18% de los pacientes. Conclusiones: La incidencia de Hipotiroidismo congénito en recién nacidos prematuros en el Hospital Infantil de Morelia es de 1.98 por cada 100 hospitalizados en el servicio de cuidados intensivos neonatales. Se observó el que los casos confirmados de Hipotiroidismo congénito fueron en el grupo de recién nacidos prematuros tardíos de 36 semanas de gestación, así como los de peso bajo para edad gestacional. .
