Intervenciones personalizadas de cuidado enfermero en personas con presencia de fistula entero-cutánea

Abstract

The nurses’ process is a systematic, humanistic, dynamic, flexible, interactive and dynamic method who bring the care of the real or potential problems in the individual, familiar o community responses, getting scientific foundation of nursing. In that way, the professional of this discipline performs an indispensable role in the execution of the health caring who are provide inside and outside of health centers. Objective. Perform a personalized caring plan to a patient with enterocutaneous fistula emerged as a compilation of post-operatory open cholecystectomy and abdominal sepsis. Method. A systematic analyze of the presented post-operatory condition were done to give theoretical support to the ailing, also, an individual valuing getting the valuation guide of the Macro Theory of self-care deficit of Dorothea E. Orem as a base; finally, the North American Nursing Diagnosis Association (NANDA), Nursing Interventions Classification (NIC) and Nursing Outcomes Classification (NOC) were the starting point to effect an analyze, selection and priorization of the identified nurses’ diagnosis and the application of the care plan. Results. A caring plan was executed from the six prioritized diagnosis where it was achieved keeping the nutritional intake from the parental nutrition ministration. Using the Orem’s proposal compensatory care was offered when the patient's respiratory status was treated and keeping patent airway as well as fluid balance and timely wound care. Conclusions. The Personalized Caring Plan strengthens the care that the nurses’ professional give as an individualized way basing the presented necessities of every single patient contributing to get improve the wellness and health of people.

El Proceso Enfermero (PE) es un método sistemático, humanista, intencionado, dinámico, flexible e interactivo que permite brindar los cuidados ante problemas reales o potenciales centrados en las respuestas humanas del individuo, familia y comunidad, además, con él se fundamenta científicamente la Enfermería. En ese sentido, el profesional de esta disciplina desempeña un rol indispensable en la ejecución de los cuidados que se proporcionan tanto dentro como fuera de los centros de salud. Objetivo. Realizar un Plan de Cuidados Personalizado (PCP) a usuaria con fístula entero-cutánea surgida como complicación pos-operatoria de colecistectomía abierta y sepsis abdominal. Método. Se realizó una revisión sistemática de la condición pos-operatoria presentada para dar sustento teórico de dicho padecimiento, se procedió a realizar la valoración individual a partir de la guía de valoración de la Macro Teoría del Déficit de Autocuidado de Dorothea E. Orem; partiendo de la taxonomía North American Nursing Diagnosis Association (NANDA), Nursing Interventions Classification (NIC) y Nursing Outcomes Classification (NOC), además, de efectuar el análisis, selección y priorización de los diagnósticos de enfermería identificados, así como la aplicación del plan de cuidados. Resultados. De los 6 diagnósticos priorizados se ejecutó un plan de cuidados y se logró mantener la ingestión de nutrientes por la ministración de nutrición parenteral. De acuerdo a la propuesta de Orem se brindó cuidado compensatorio al atender el estado respiratorio de la paciente y mantener una vía aérea permeable, así como el equilibrio hídrico, con un cuidado oportuno de la herida. Conclusiones. El PCP fortalece el cuidado que el profesional de enfermería otorga de manera individualizada basándose en las necesidades presentadas por cada paciente, contribuyendo en la salud y bienestar de la misma.
