Diagnóstico y tratamiento de linfosarcoma en gatos

Abstract

Lymphosarcoma (LSA) is the hematopoietic tumor that most commonly affects cats. It is one of the neoplasms that have the best response to antineoplastic chemotherapy; unfortunately, patients eventually succumb to the dissemination of the disease. The etiology of lymphosarcoma in small species is largely unknown. Certain varieties of lymphosarcomas in cats have been associated directly or indirectly with feline leukemia virus (FLV) and feline immunodeficiency virus (FIV), respectively. Various classification schemes for lymphosarcoma have been proposed, including those based on the affected site, World Health Organization (WHO) clinical staging, histologic/cytologic phenotype, and immunophenotype. Cats that do not receive treatment, once the diagnosis of ALS has been established, have an average life span of 4 to 6 weeks. Depending on the protocol is the survival expectancy, in general monotherapy with prednisone or doxorubicin or their combination offer good quality of life for short periods, and the most complex treatments are those that offer up to two years of life, but also have more associated complications to the cytotoxicity of chemotherapeutic agents, in addition to the cost of the treatments. The clinical recognition of lymphosarcoma in cats, in the early stages, allows proposing treatments with cytotoxic drugs that offer a good time and quality of life for the patient, and that are also acceptable to the owner. In the present work, a review of the literature on the diagnosis and treatment of lymphosarcoma in cats is offered, with the intention of serving as a support to the student and Veterinary Zootechnician with an interest in the clinic of small species.

El linfosarcoma (LSA) es el tumor de origen hematopoyetico que más comúnmente afecta al gato. Es de las neoplasias que tienen mejor respuesta a la quimioterapia antineoplásica, desafortunadamente los pacientes eventualmente sucumben a la diseminación de la enfermedad. La etiología del linfosarcoma en pequeñas especies en su mayor parte es desconocida. Ciertas variedades de linfosarcomas en gatos han sido asociadas directa o indirectamente, con el virus de la leucemia felina (LeVF) y virus de la inmunodeficiencia felina (VIF) respectivamente. Se han planteado varios esquemas de clasificación del linfosarcoma, incluyendo el basado en el sitio afectado, estadio clínico de la Organización Mundial de Salud (OMS), el fenotipo histológico/citológico, y el inmunofenotipo. Los gatos que no reciben tratamiento, una vez establecido el diagnostico de LSA, tienen un promedio de vida de 4 a 6 semanas. Dependiendo del protocolo es la expectativa de sobrevida, en general la monoterapia con prednisona o doxorrubicina o su combinación ofrecen buena calidad de vida por periodos cortos, y los tratamientos más complejos son los que ofrecen hasta dos años de vida, pero también presentan más complicaciones asociadas a la citotoxicidad de los quimioterapeuticos, además del costo de los tratamientos. El reconocimiento clínico del linfosarcoma en gatos, en los estadios tempranos, permite proponer tratamientos con medicamentos citotóxicos que ofrezcan un tiempo y calidad de vida buenos en el paciente, y que además sean aceptables para el propietario. En el presente trabajo se ofrece una revisión de literatura sobre el diagnóstico y tratamiento del linfosarcoma en gatos, con la intención de que sirva de apoyo al estudiante y Médico Veterinario Zootecnista con interés en la clínica de las pequeñas especies.
