Comparación y estimación de la tasa de filtración glomerular mediante la fórmula de Cockcroft-Gault y fórmula MDRD, en pacientes mayores de 60 años

Abstract

Renal function is defined by the glomerular filtration rate (GFR). Must be greater than 60 ml/min/1, 73 m2 in subjects who do not suffer from kidney disease 70. The Cockcroft -Gault formula should not be used to estimate GFR in the elderly since the underrated them, be calculated by the MDRD formula. In clinical practice there is increased use of the Cockcroft -Gault is to see if using this formula really underestimates these values in relation to other forms of assessment of GFR. OBJECTIVE. To estimate and compare the results of GFR according to the Cockcroft -Gault , MDRD formula and keratinize clearance in 24hrs. MATERIAL AND METHODS. It was a descriptive studio, observational, comparative. Through random probability sampling. We measured serum creatinine and urine creatinine clearance of 24 hrs. GFR was calculated using the Cockcroft -Gault and the MDRD. We compared the results. RESULTS. We obtained an underestimation of GFR in these patients using the Cockcroft Gault, compared to that obtained by the other two methods. CONCLUSIONS. It was concluded that for these patients is better to use the MDRD formula to obtain a more reliable TFG.

La función renal se define por la tasa de filtración glomerular (TFG). Debe ser superior de 60 ml/min/1,73 m2 en sujetos que no sufren enfermedad renal a partir de los 70 años. La fórmula de Cockcroft-Gault no debe usarse para estimar la TFG en ancianos ya que la subestima; en ellos, debe calcularse mediante la fórmula MDRD. En la práctica clínica se observa la mayor utilización de la fórmula de Cockcroft -Gault se quiere observar si realmente mediante esta fórmula se subestiman dichos valores en relación a las otras formas de valoración de la TFG. OBJETIVO: Estimar y comparar los resultados de la TFG de acuerdo a la ecuación de Cockcroft-Gault, la formula MDRD y depuración de creatinina en 24hrs. MATERIAL Y MÉTODOS: Fue un estudio descriptivo, observacional, comparativo. A través de muestreo probabilístico aleatorio. Se midió creatinina sérica y la depuración de creatinina en orina de 24 horas. Se calculó la TFG por medio de la fórmula de Cockcroft-Gault y por la MDRD. Se compararon los resultados. RESULTADOS. Se obtuvo una subestimación de la TFG en estos pacientes utilizando la fórmula de Cockcroft Gault, respecto a la obtenida por los otros 2 métodos. CONCLUSIONES. Se concluyó que para estos pacientes es mejor utilizar la fórmula de MDRD para obtener una TFG más fiable.
