To determine the incidence of Myasthenia Gravis by age, sex and type in children of the Children's Hospital of Morelia in a period of 6 years, to identify the main clinical manifestations, laboratory and image alterations in the children studied, to describe the treatment and evolution of the cases studied, design a proposed diagnostic protocol for the child with MGJ. MATERIAL AND METHODS: Observational, analytical and retrospective study. Information was obtained from the clinical files of pediatric patients with MG from the pediatric neurology service at the Children's Hospital of Morelia, where a total of 15 patients were found, 13 met the inclusion criteria and 2 were excluded. RESULTS: The MGJ was observed in 60% in the male sex and 40% in the female, in the prepubertal stage a 1.2: 1 relationship was observed, predominantly the male sex and in the postpubertal stage a 3: 1 ratio with female predominance, fashion was 2 years whereas the median was 6.6 years, according to the Osserman classification the most frequent clinical form was the generalized mild (IIa) in 61%, the most frequent symptoms were: palpebral ptosis, muscular fatigability and hypotonic. The Anti-AchR antibodies were positive in 39%, no alterations were demonstrated in the thymus, the most used treatment was pyridostigmine, steroid and intravenous immunoglobulin, the evolution after the treatment was: asymptomatic in 54% of the patients. CONCLUSIONS: MGJ presents equitably in the prepubertal stage in both women and men, however, in the postpubertal stage it is predominantly female, according to the Osserman classification and the form of mild generalized presentation predominates. MGJ is complicated due to the limited tolerability and limited availability in pediatric patients, which continues to be a challenge.
Determinar la incidencia de Miastenia Gravis por edad, sexo y tipo en los niños del Hospital Infantil de Morelia en un periodo de 6 años, identificar las principales manifestaciones clínicas, alteraciones de laboratorio e imagen en los niños estudiados, describir el tratamiento y evolución de los casos estudiados, diseñar una propuesta de protocolo diagnóstico del niño con MGJ. MATERIAL Y MÉTODOS: Estudio observacional, analítico y retrospectivo. Se obtuvo información de los expedientes clínicos de los pacientes pediátricos con MG del servicio de neurología pediátrica en el Hospital Infantil de Morelia, encontrándose un total de 15 pacientes, 13 cumplieron con los criterios de inclusión y se excluyeron 2. RESULTADOS: La MGJ se observó en 60% en el sexo masculino y 40% en el femenino, en la etapa prepuberal se observó una relación 1.2:1 predominando el sexo masculino y en la etapa postpuberal una relación 3:1 con predominio femenino, la moda fue de 2 años mientras que la mediana de 6.6 años, según la clasificación de Osserman la forma clínica más frecuente fue la generalizada leve (IIa) en un 61%, los síntomas más frecuentes fueron: ptosis palpebral, fatigabilidad muscular e hipotonía. Los anticuerpos Anti-AchR fueron positivos en 39%, no se demostraron alteraciones en el timo, el tratamiento más utilizado fue piridostigmina, esteroide e inmunoglobulina intravenosa, la evolución posterior al tratamiento fue: asintomáticos en 54% de los pacientes. CONCLUSIONES: La MGJ se presenta de manera equitativa en la etapa prepuberal tanto en mujeres como en hombres, sin embargo, en la etapa postpuberal tiene predominio por el sexo femenino, según la clasificación de Osserman predomina la forma de presentación generalizada leve, el diagnóstico de la MGJ es complicado debido a la poca tolerabilidad y disponibilidad limitada en los pacientes pediátricos, por lo cual continúa siendo un reto.
